What is High Resolution Anoscopy (HRA)?

HRA is a diagnostic procedure using a high-powered microscope to detect anal pre-cancerous cells caused by human papillomavirus (HPV). This is an intervention recommended for EARLY detection of anal cancer. Anal dysplasia is a term used to describe changes at the cell level on the anal lining that can progress into anal cancer.

The International Anal Neoplasia Society (IANS) recommends yearly anal cancer screening to the following groups:

People living with HIV:

  • Men who have sex with men and transgender women starting at age 35 years old.
  • Men who only have sex with women and cis-women: starting at age 45.

People without HIV:

  • Men who have sex with men and transgender women without HIV: starting at age 45
  • Any women with diagnosis of high-risk cervical dysplasia or cervical cancer: within 1 year of cervical dysplasia/cancer diagnosis.

There are few people certified in the state of Texas to perform HRA. In our clinic, Ryan Lomeda, FNP-C, AAHIVS, is certified to perform HRA and treat HSIL.
